There are plenty of women who can't go get laid whenever they want, or can't do so safely. Women look at porn just as much as guys do, and having regular sex doesn't eliminate the desire for porn either. Otome games are usually romance games not JUST sex (unfortunately almost none of them even have sex), in this game you spend a lot of time with the guys building your friendship and relationship with them. That kind of romance and emotional bond can't be generated IRL so easily or with just anyone.
Women IRL have the same chance of getting a harem of hot, perfect, totally-into-her dudes as men do (i.e. none).
